In the form of surcharge and GST, 1.18% is lost. For delivery charges, it depends on the denomination and number of cards you buy. The delivery charge for each order is Rs.80 or Rs. 100 depending on the location. If you buy one Rs. 10000 card (rare to get), you loose 1% on delivery

Such cards are mostly used for rotating money. Or if you don't have good offline rewarding card.

If you have SBI cashback, you can buy 10K voucher at 5% discount and 1% delivery charge. When you use it offline, it will give you 4% return on spends, which is usually better than most cards out there.

Also the means to use Amex cards offline (Amex Plat travel) via this route
 
@Inert Is this card accepted at all POS machines or any mcc is excluded. Planning to use it in hospital in the coming week. Please suggest

Some PoS machines may not work. I have done transactions in about 10 different merchants. It has failed couple of times. The failure could be due to the fact that the machine is configured not to accept such cards or card is not read properly due to poor quality magnetic strip. In the later case, you can try a few times. It would work.

It’s a Paytm POS machine. Any idea on this ?
